New Patient Forms

We kindly ask all new patients to download and complete the new patient forms prior to their scheduled appointment.
Bringing your completed forms to the clinic helps streamline the check-in process and ensures you receive the most thorough and efficient care during your visit.

Please make sure all sections are filled out as thoroughly as possible.

 
Alternatively, if you prefer to complete the new patient forms in the clinic, we recommend arriving at least 30–40 minutes prior to your scheduled appointment to allow sufficient time for paperwork and to help ensure your visit begins on time.

Additional Information

Please remember to bring the following to your appointment:

• A physical copy of your medical insurance card

• Your driver’s license

• Any documents you would like Dr. Lee to review

To help ensure a more thorough and smoother visit, please bring the requested items with you.
If you do not have a physical copy of your insurance card, be sure to print a paper copy and bring it to your appointment.
If you’re unable to do so, we kindly ask that you reschedule your appointment in advance.
